Blakeford Senior Life’s Continuing Care at Home Program Expands


LiveWell on the Mountain is now accepting new members

Blakeford Senior Life — a nonprofit provider of senior lifestyle solutions headquartered in Nashville — has expanded its Continuing Care at Home (CCaH) program, LiveWell By Blakeford, into Sewanee, Tenn. LiveWell on the Mountain is an expertly managed and personally delivered plan for aging at home available to adults over 60 in Sewanee and Monteagle.

Combining the best aspects of a long-term care plan, home care and retirement community living in an innovative package of benefits, LiveWell allows older adults to plan and invest in their futures while they are healthy and independent. The program is for adults who want to be proactive and establish a plan that will ensure they have long-term care coverage at home for a lifetime.

“After seven years of providing our CCaH program to Nashville and its surrounding counties, we are thrilled to welcome our first members in a new area of Tennessee through LiveWell on the Mountain,” said Jane Kelley, executive director of home and community services at Blakeford. “Our state’s population of those over 60 is growing steadily and will continue to climb. To serve this demand, Blakeford believes long-term care planning should be personalized to each person’s needs and desires. Expanding to another storied town with a leading university and robust older population means we can offer a unique, top-quality option to those who want to plan to stay at home as they age.”

LiveWell on the Mountain is collaborating with Folks at Home, a Sewanee-based non-profit dedicated to empowering individuals to live at home with dignity in the community they love, to provide services and offer joint programming and events to their shared members. Every LiveWell on the Mountain member will receive a complimentary Folks at Home membership.

Featuring a holistic approach to aging, LiveWell is designed to eliminate the stressors that aging can place on older adults, their spouse or their family. The LiveWell team handles care coordination for its participants and takes a proactive approach toward preventing issues through home safety assessments, nutritional counseling, fitness programming, patient advocacy and more. LiveWell care coordinators are available to problem solve around urgent needs, assist with daily tasks such as transportation to the doctor and offer medical guidance and education.

The LiveWell on the Mountain team welcomed its first 10 members with a luncheon at the Sewanee Inn on Feb. 24. Along with the new members, Blakeford Senior Life’s president and CEO Brian Barnes, Jane Kelley and other team members from Folks at Home and LiveWell attended the event.
